HPM's properties, please use the link below. The link
will download a .pdf file
that you can print and fill out by hand. You must fill out the application completely
to be considered. Be sure to sign where indicated on the application.
We cannot process unsigned or incomplete applications, or applications missing documents.
Please submit the following along with the application form:
- Proof of all income listed on application
(paycheck stubs from the last month if you work, if you receive assistance we need proof of assistance)
- Two forms of ID we can photocopy (one must be a government issued ID with a picture)
- $30.00 per application fee. Money Orders only.

Each person 18 years or older who would be living in a unit must submit
an application in person at our office. This includes adult children still living at home.
Please call HPM for more information about the application procedure for adult children.
Please note: we do require masks to view a vacant unit and in our office. You must
bring your own mask.
You MUST HAVE AN APPOINTMENT TO SUBMIT AN APPLICATION. The application process can take from three to
five days to process all applications and obtain the necessary information.
